Please fill out the Registration form clearly and completely.   Be sure to sign the form.
A separate registration form must be used for each person registering.

Deadline for class registration is October 5, 2007.
Deadline for pre-enrollment is September 15, 2007.

NOTES REGARDING PAYMENT METHODS:

		If paying by check, please include your check, payable to Ohio LongArm Quilters, for full payment with your
		registration form. Personal checks returned to OLAQ for any reason will incur a $25 fee.

		If paying by credit card, send your registration form and we will invoice you via email. You will be directed to the
		PayPal website where you can securely pay online by entering your credit card information. No PayPal account is required. You MUST include your email address on the registration form.

		If paying through PayPal, send your registration form and we will invoice you via email. You will be directed to the
		PayPal site where you can securely pay online through your PayPal account.

IMPORTANT INFORMATION

		Please include your email address on the registration form. (This is required if you want to pay by credit card or
		PayPal.) We will send you a confirmation of your registration via email. If you do not have email, you will receive a paper confirmation via US Mail.

		Scheduling of classes will be done on a first come, first served basis. We will indicate when classes are full on
		the class schedule webpage. If you sign up for a class that is full, a representative of OLAQ will contact you by email or phone to discuss your options.

		Boxed lunches are available on both Friday and Saturday. They are $8.00 each and must be pre-ordered on your
		registration form.

		Tickets for the Friday evening dinner banquet can be ordered on your registration form. You do NOT need to
		register for classes to attend the banquet. Deadline for purchasing tickets is October 5, 2007. See the Banquet page on the website for other ways to purchase tickets.

		Raffle tickets for the Feathered Fantasy quilt can be ordered on your registration form. Tickets ordered this way will
		be distributed to the registrant upon check in at the registration desk as the event. See the Raffle Quilt page for others ways to purchase tickets.

		Registration fee is $30 per person. This includes processing fees and a name badge. This fee is NOT
		refundable.

		Registration forms received by September 15, 2007, are considered pre-enrolled. Registrants who are pre-
		enrolled will receive a Quilting with Machines logo apron and other goodies which will be distributed during check-in at the registration desk at the event.

		To change a class after your registration form has been processed, contact Registrar Pat Waldron at
		registrar@longarmohio.com. OLAQ will permit changes through September 15, 2007. After September 15, 2007, each class change will incur a $5 fee. Adding classes to your schedule can be done (even at the event) at any time for no extra fee if there is space available.

		To cancel your classes after your registration form has been processed, mail a copy of your confirmation along
		with a written request to cancel. If this is received before August 15, 2007, you will receive a full refund of class fees. If received on or after August 15, 2007 through September 15, 2007, you will receive a 50% refund of class fees. After September 15, 2007, NO refunds will be made. However, you may sell your classes to another person registered for the event. There will be a bulletin board at Quilting with Machines where you can post a notice that you have class(es) to sell. You must notify Registrar, Pat Waldron of any classes you have sold to another person.

		The Ohio LongArm Quilters reserves the right to cancel a class if fewer than 10 students sign up. OLAQ will notify
		you of any canceled classes that you have already signed up for. You will have the option to select another class or be issued a refund.

LIABILITY WAIVER AND AGREEMENT OF
INDEMNIFICATION

Your registration and attendance is in agreement to the
Liability Waiver and Indemnification Agreement as follows:
